FURRY BLISS MOBILE SPA SERVICE AGREEMENT

1. FURRY BLISS MOBILE SPA will always do what is safe and humane for your pet. I understand that a matted coat can be dangerous and painful for my pet and must be removed by shaving. I authorize FURRY BLISS MOBILE SPA to shave my pet at their discretion and when necessary.

2. FURRY BLISS MOBILE SPA will always handle your pet with care. Unfortunately, accidents can happen when working with pets. FURRY BLISS MOBILE SPA is not responsible for injuries caused by pre-existing conditions and must be made aware of any medical conditions, allergies, or previous aggression before the pet's appointment. FURRY BLISS MOBILE SPA has the right to refuse service if the safety of the pet or groomer may be compromised.

3. FURRY BLISS MOBILE SPA requires 48 hours notice for cancellations. A cancellation without 48 hours notice will require a $30 fee due before rebooking the next appointment.

4. In the case of extreme weather, including temperatures that are extremely low or high, FURRY BLISS MOBILE SPA may have to reschedule your appointment for the safety of pets and groomers.

5. Current up-to-date rabies information is required for all pets before any service is started. Paperwork must be sent at least 24 hours before the appointment along with a signed service card.

6. I authorize FURRY BLISS MOBILE SPA to take pictures of my pet during their appointment.

7. FURRY BLISS MOBILE SPA is authorized to use an outdoor outlet on the premises during your pet's groom. This is the most efficient way to keep equipment running smoothly during the groom.

8. FURRY BLISS MOBILE SPA will do its best to arrive on time for your appointment. Due to traffic or unforeseen circumstances with a previous appointment, FURRY BLISS MOBILE SPA may arrive within the hour.
